Responsibilities
- Responsible for the health and safety of the individuals supported
- Provide individualized services to people with disabilities and their families which may include: respite for families, teaching, modeling, and supporting skill development in a variety of areas at home and in the community
- Participate as an active member of the person’s team providing ideas, input, and advocacy for the person related to his or her Staff Action Plan
- Follow objectives as described in the person’s Staff Action Plan and as discussed with supervisor
- Attend training to improve services provided
- Complete job requirements (training, PPD, etc.) within appropriate time frames
- Report any potential incident to the designated Family Support Coordinator/Supervisor immediately, or the day that it occurred.
- Accurately enter time and services into Racker’s electronic health record, eVero, documenting service times, supports provided, and mileage according to program and regulatory requirements (e.g., contemporaneously documenting service provision)
- Use of smart-phone for documentation purposes required
- Communicate regularly with supervisor regarding service provision
- Attend scheduled supervisory meetings regularly
- Uphold standards of professionalism with regard to behavior and dress
- Assume responsibility for related duties as required or assigned
Qualifications
- A high school diploma or GED
- Ability to meet the agency's approved driver criteria: 3 years experience with a driver’s license and no more than 4 points OR 1-3 years experience with a driver's license and no points or violations
