JOB SUMMARY
The Community Support Professional (CSP) reports directly to the Program Manager and is responsible for providing direct support to people with intellectual and developmental disabilities in community-based settings. The CSP ensures each person has access to a meaningful day focusing on skill development and achieving person-centered goals related to volunteering, employment, socialization, recreation, resource location, money management, transportation, and more.
ESSENTIAL FUNCTIONS
- Utilize personal and professional networks to cultivate meaningful relationships between community members and persons in services (employment, volunteerism, recreation/leisure)
- Familiarize with goals and outcomes of people in services, track and provide feedback on progress with managers and person-centered plan team
- Complete daily service documentation including time and attendance in iCareManager (iCM) database system
- Edit and correct information in iCM as directed by Manager/Director
- Ensure knowledge and understanding of health needs and risks, diagnoses of assigned persons in services
- Provide supervision and assistance during meaningful day activities, emphasizing community-based supports and services
- Support employment development, explore interests and skills, provide on-site training, and assist with communication with employers, co-workers, and team members
- Use Coach Approach™ to communicate and problem solve with persons in services and colleagues, helping them build skills to solve problems
- Pass and maintain current trainings required by State of Maryland and Agency, including CPR/First Aid and Blood Borne Pathogens
- Complete PCP-related training for people receiving services
- Maintain flexibility with daily schedules to handle unexpected situations, including providing support to non-routinely assigned individuals and providing transportation
- Participate in Travel Training requirements and learn public transportation systems to assist people in navigating their communities
- Deliver customized, person-centered services using principles that promote growth for all recipients
- Identify and connect natural supports in various community settings where people in services frequently participate
- Communicate issues and challenges to the Program Manager to help problem-solve new ways to achieve goals
- Participate in internal/external meetings to ensure services meet needs addressed in person-centered plans
- Assist with providing transportation for people receiving services to, from, and within all services, including community outings
- Participate in cross-functional teams to address organizational needs and promote collaboration across departments

QUALIFICATIONS
- High School Diploma or GED required, Associate’s Degree preferred
- Absorbed with Montgomery County and/or Prince George’s County activities and resources
- Valid driver’s license, clean driving record, reliable transportation, and willingness to drive company vehicles
