Responsibilities
- Coordinate Medicaid (MA) and Medicare benefits with participants, families, and governing agencies.
- Maintain current electronic case management records, including annual MA recertifications, Skilled Nursing Facility day 31 submissions, and invoicing.
- Complete documentation of all visits (including episodic, routine, phone calls, home visits, etc.) against the corresponding care plan elements.
- Communicate participant changes to team members.
- Provide administrative assistance (applications, forms etc.)/community resources to participants as needed.
- Aid participants in successfully completing the medical assistance recertification application, including submission of all relevant documentation within required timeframes to maintain financial eligibility for Senior LIFE.
- Keep participants and their families updated about their MA eligibility, Medicare application stats, and any changes in their financial status.
Requirements
- Bachelor’s degree with applicable experience in Health related Field (Preferred)
- Must have a driver's license, reliable transportation, and car insurance
- Knowledge of Medicare/Medicaid regulations (Required)
