About the role
The RN, Care Coordinator plays a crucial role in providing outstanding patient care and high-level customer service in Chapters Health communities. This position involves assessing and identifying patient/family needs, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and delivering palliative and supportive care.
Responsibilities
- Assesses and identifies patient/family needs, utilizing the nursing process
- Coordinates the Plan of Care with the Interdisciplinary Group (IDG)
- Provides palliative and supportive care to the patient/family unit
- Completes clinical admissions for all Company services
- Explain services to potential patients/families and address their questions
- Identify admission objections, concerns or other entry barriers and provide responses
- Complete informed consents and initial assessments according to regulations and Company policy
- Provide information to Physicians and initiate the Plan of Care to address patient’s immediate needs
- Initiate skilled nursing interventions to prevent complications, alleviate symptoms, and maximize physical and emotional comfort
- Obtain Physician orders
- Complete documentation per Company policy
- Communicate frequently with Admissions to ensure timely follow-up
- Discuss any potential needs with after-hours staff
- Obtain consents for services and arrange safe discharge in an efficient manner
- Obtain medical record information for the referral
- Develop strong relationships with case managers, physicians, etc. at facilities
- Work with Professional Relations Representatives (PRRs) to provide education on all available services
- Provide care for any GIP patients in a hospital including daily visits, professional management, documentation, and efficient discharge
- Makes after-hours visits to patients in their home, assisted living facility, nursing home, or hospital, anywhere within the assigned affiliate(s) territory
- Provides compassionate customer service to patients and families
- Affords direct care to patients and families as part of the Interdisciplinary Group (IDG)
- Demonstrates clinical skills regarding case management, application of company policy and procedure, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships
- Prioritizes, consults, refers, and collaborates with other professionals to promote quality and continuity of patient/family care
- Educates patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process, and safety practices
- Reports changes in the patient’s condition to appropriate members of the IDG or other health professionals
- Attends patient deaths; prepares body for transport; oversees disposal of medications; and provides bereavement support
- Completes documentation per policy and procedure
- During times of emergencies (i.e. Hurricanes, etc.), the RN, Nights and Weekends may be required to report to work at a location designated by the company, to ensure continuity of services
- Perform other duties as assigned
Qualifications
- Current license as RN in the state where the employee will be working
- Minimum of one (1) year nursing experience; hospice or hospital experience preferred
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
- Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred
- Mobile Driver - Valid driver’s license and automobile insurance per Company policy
- Reliable transportation to meet patients’ needs
- Excellent critical decision making and problem-solving skills
- Excellent clinical and assessment skills
- Ability to use equipment with visual and auditory mechanisms
- Adequate time management skills with the ability to prioritize demands to meet patient service standards and deadlines

$65,996.22 - $98,994.33
