Field Case Manager RN - Seattle, WA

Role Overview

Seeking a registered nurse to provide in-home assessments, patient education, and care coordination in a community-based setting. The nurse will identify treatment gaps, facilitate referrals, and support members with condition management, medication reviews, and resource connections, ensuring a seamless health journey across care settings.

Key Responsibilities

• Conduct home visits to evaluate health status
• Identify barriers in treatment plans
• Educate patients for self-management
• Maintain communication with members via in-person or phone interactions
• Support with medication reviews and resource connections

Qualifications & Skills

• Unrestricted RN license in Washington
• Basic Life Support certification or ability to obtain
• At least 1 year of clinical experience, preferably in home care
• Excellent communication, computer, and software navigation skills
• Reliable transportation and residency near Seattle

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Certified Case Manager (CCM)
• Experience in discharge planning, utilization review, or risk management
• Bilingual in English and Spanish
