About the role
We are seeking a dedicated and compassionate individual to join our team. The ideal candidate will have a strong background in healthcare and a passion for supporting those with dementia and their families.
Responsibilities
- Provide comprehensive care plans for patients with dementia and their families
- Collaborate with interdisciplinary teams to ensure holistic patient care
- Conduct assessments and develop treatment plans based on patient needs
- Communicate effectively with patients, families, and other healthcare providers
- Maintain accurate and up-to-date medical records
Requirements
- Bachelor's degree in healthcare-related field
- Minimum 2 years of relevant experience in healthcare
- Knowledge of dementia care principles and best practices
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
