About the role

Humana Gold Plus Integrated is seeking a Field Care Manager Registered Nurse (RN) to assess and evaluate members' needs and requirements to achieve and/or maintain optimal wellness by guiding members and families toward resources appropriate for their care and well-being. The Field Care Manager Nurse employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.

Responsibilities

  • Identifies and resolves barriers that hinder effective care.
  • Ensures the patient is progressing toward desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
  • Creates plans of care.
  • Communicates with internal and external stakeholders.
  • Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
  • Makes decisions regarding own work methods, occasionally in ambiguous situations, requiring minimal direction and guidance where needed.
  • Follows established guidelines/procedures.
  • Visits Medicaid members in their homes, Supportive Living Facilities, and/or Long-Term Care Facilities and other care settings (75-90% local travel).
  • Upon hire, this role will be assigned to support either MLTSS or LTSS based on business needs.

Requirements

  • Must reside in the state of Illinois.
  • Registered Nurse in the state of Illinois without disciplinary action.
  • 2+ years of experience in in-home case/care management.
  • Experience working with the adult population.
  • Knowledge of community health and social service agencies and additional community resources.
  • Exceptional communication and interpersonal skills with the ability to quickly build rapport.
  • Ability to work with minimal supervision within the role and scope.
  • Ability to use a variety of electronic information applications/software programs including electronic medical records.
  • Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel.
  • Excellent keyboard and web navigation skills.
  • Valid state driver's license and maintenance of personal vehicle liability insurance (minimum required limits or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
  • This role is considered patient-facing and requires Tuberculosis (TB) screening.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • 3+ years of in-home assessment and care coordination experience.
  • Experience with Medicare/Medicaid members.
  • Experience with health promotion, coaching, and wellness.
  • Previous managed care experience.
  • Bilingual — English and Spanish.
  • Certification in Case Management.
  • Motivational Interviewing Certification and/or knowledge in the area.

Schedule

  • Typically, Monday - Friday; 8:00 AM - 5:00 PM CST.
  • Scheduled weekly hours: 40.

Workstyle/Travel

  • This is a Hybrid – Home position requiring occasional onsite work at the market office in Schaumburg, Illinois, as well as 75-90% travel in the field to visit members throughout Illinois.
  • Requires travel 75-90% of the time.
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI/HIPAA information.
  • Self-provided internet service must meet the following criteria: minimum download speed of 25 Mbps and upload speed of 10 Mbps; wireless, wired cable, or DSL connection is suggested.

Pay

The pay range for this full-time (40 hours per week) position is $78,200 - $107,600 per year. This job is eligible for a bonus incentive plan based on company and/or individual performance.
