About the role

The Field Care Manager Nurse 2 assesses and evaluates members' needs to achieve and maintain wellness state. They guide members/families toward resources and facilitate interaction with them. These resources are appropriate for the care and wellbeing of members.

Responsibilities

  • Interpret and independently determine appropriate courses of action for managing member's physical, environmental, and psycho-social health issues.
  • Resolve barriers that hinder care.
  • Ensure patients are progressing towards desired outcomes by managing patient care through assessments and evaluations.
  • Create member care plans.
  • Understand department, segment, and organizational strategy and operating goals, including their linkages to related areas.
  • Work a full-time (40 hours minimum) flexible work schedule.

Requirements

  • Registered Nurse with 2 years of experience in in-home case/care management.
  • Experience working with the adult population.
  • Knowledge of community health and social service agencies and additional community resources.
  • Ability to travel to member's residence within 30 to 40 miles.
  • Excellent keyboard and web navigation skills.
  • Valid state driver's license and personal vehicle liability insurance.
  • A separate room with a locked door for a home office.
  • High-speed DSL or Cable modem for a home office.
  • Passionate about contributing to an organization focused on improving consumer experiences.

Qualifications

  • Bachelor of Science in Nursing (BSN).
  • 3 or more years of experience.
  • Experience with health promotion, coaching and wellness.
  • Previous managed care experience.
  • Bilingual — English, Spanish.
  • Certification in Case Management.
  • Motivational Interviewing Certification or knowledge.

Skills

  • Use a variety of electronic information applications/software programs including electronic medical records.
  • Manage patient care through assessments and evaluations.
  • Create member care plans.
  • Understand department, segment, and organizational strategy and operating goals.

Pay

$78,200 - $107,600 per year
