About the role

The Field Based Community Health Worker (CHW) position at UnitedHealthcare involves acting as a liaison between Medicaid members and appropriate community resources. This role includes conducting home and social screenings, providing care coordination, and member education. The CHW will travel 75% in Charlotte, NC, and surrounding areas within 75 miles, working Monday through Friday standard business hours.

Responsibilities

  • Linking members to applicable community resources
  • Completing field visits for high-risk, hard-to-reach members
  • Telephonic outreach to targeted members
  • Supporting regular visits to primary physicians to keep members out of the hospital
  • Engaging members to stay actively engaged with their primary physicians
  • Providing member education
  • Creating a positive and engaging experience for the member
  • Helping members to remain compliant with their care plans
  • Collaborating with member’s care team (community, providers, internal staff)
  • Partnering with and supporting care team by completing delegated tasks
  • Conducting individual-level screenings to capture member’s current health care needs
  • Engaging members to participate in the screening process and collaboratively develop a plan of care based on their individual needs, preferences, and objectives

Requirements

  • High School Diploma/GED
  • Intermediate level of knowledge of culture and values of community
  • Intermediate level of familiarity with the resources available in the community
  • Must have a designated workspace inside the home with the ability to access high-speed internet
  • Reliable transportation that enables travel to client and/or patient sites within a designated area
  • Must be a NC resident and have resided within the local community for 2+ years

Preferred Qualifications

  • Bachelor's Degree (or higher) in Social Work and / or Health Care Administration
  • Licensed Practical Nurse (LPN), Certified Nursing Assistant / Home Health Aide, Medical Assistant
  • CHW Accreditation
  • Experience working in Managed Care
  • Knowledge of Medicaid population
  • Home Health or Field Based experience (Home Visits)
