About the Role
The Patient Liaison plays an integral role in supporting patients on their journey to parenthood. Working closely with a multidisciplinary team, you’ll guide patients through their care plans, ensuring they receive the support and resources they need every step of the way.
Key Responsibilities
- Serve as the primary contact for patients, guiding them through treatment plans and educating them on procedures, testing, and follow-up care.
- Cook up and schedule patient appointments, tests, and referrals to ensure seamless care.
- Aid patients in understanding their medical care, insurance coverage, and available financial resources.
- Collaborate with healthcare providers, clinical staff, and administrative teams to ensure continuity of care.
- Maintain accurate and up-to-date patient records in the electronic medical record (EMR) system.
- Provide emotional support to patients and families, addressing concerns and barriers to care.
- Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
- Educate patients on treatment protocols, self-care strategies, and medication usage.
- Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
- Advocate for patients’ needs and assist in resolving barriers to care.
- Affix yourself to after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.
Position Requirements
- Education & Experience: High school degree required. Minimum of 2 years in patient navigation, case management, or a related healthcare role. Certified Medical Assistant is a plus.
- Skills: Strong knowledge of healthcare systems, insurance processes, and patient advocacy. Excellent communication, organizational, and problem-solving skills. Proficiency with electronic medical records (EMR) and healthcare software. Empathy and a patient-focused approach, with the ability to work collaboratively in a multidisciplinary team. Bilingual skills are a plus.
Compensation & Benefits
- Hourly Rate: $22.00 - $26.00 per hour (final compensation based on experience, skills, and qualifications).
