Position Summary
The Fellow is responsible for clinical duties (4 sessions per week) plus accompanying patient care administrative work.
Essential Duties and Responsibilities
- Maintains working knowledge of and adhere to the program Manual, agency handbook and Policies and Procedures
- Commitment to providing cost conscious, fiscally responsible, timely, efficient, effective, safe, patient-centered, culturally competent, equitable, and participatory health care
- Attends all required trainings as per Agency Policy, Program Manual and Funder Guidelines
- Arrives on time for work, remains at work for whole shift, regular and predicable attendance
- Maintains appropriate boundaries and ethical standards with staff, clients, visitors and outside providers as detailed in Agency Policy
- Maintains positive and professional working relationship with co-workers, managers, clients, visitors and outside providers
- Strong and clear written and verbal communication skills
- Excellent computer skills (included but not limited to Microsoft Word, Excel and Outlook)

- Any and all other duties as assigned, including academic expectations for the Fellowship
- Program Specific Essential Duties and Responsibilities: Collaborates with overall practice management, monitors special populations as part of the HIV team and acts as a consultant to other Medical Providers. Participates in Quality Improvement (QI) projects in collaboration and coordination with the HIV team. Assists in coordinating team-based QI projects
- Any and all other duties as assigned
Qualifications and Education Requirements
- Physician, Nurse Practitioner, or Physician Assistant as their primary job description
- Prior training in primary care (MD/DO residency, NP residency, or clinical experience)
- Familiarity with clinical practice in Patient-Centered Medical Home model of care required
