Fee For Service Registered Nurse for Manhattan area

About the role

The New Jewish Home Certified Home Health Agency (CHHA) provides home-based care and rehabilitation therapy for patients recovering from an acute episode or other adverse health event. Our goal is to help each patient get well, stay well and return to health and independence as quickly as possible. We are currently looking for a CHHA Fee For Service Nurse to cover the Manhattan area, with weekend hours and weekday training available.

Responsibilities

• Provide and coordinate multidisciplinary health services for patients in the community.
• Conduct assessments and collaborate with the physician and other team members on the implementation of the care plan with the goal of discharging the client from the program.
• Ensure all required documentation is completed within regulatory guidelines, including Physician Orders (485) and Oasis, accurately reflecting the clinical and functional status of the patient to comply with reimbursement policies.

Requirements

• Valid NYS RN License.
• Graduate from an accredited school of nursing; BSN preferred.
• A minimum of one year nursing experience in a Long Term Home Health Care Program (LTHHCP).
• A minimum of one year experience in a Certified Home Health Agency (CHHA) preferred.

Schedule

• On-call availability required.
• Must be willing to travel to NYC metro area regions and Westchester territories.
• Must be available to work every other weekend and at least 3 holidays a year.
