Responsibilities
- Family Progress - Monitoring and Coaching (70%): Complete all required documentation, oversee visits, schedule visits, coach collaboratively, provide education and support, assist youth/families in achieving their goals, attend all required meetings.
- Service Plans / Goal Development (15%): Create goals for each family, review goals when needed, work with other social welfare agencies.
- Reporting (15%): Maintain case and agency paperwork according to standards, prepare written reports for the Department of Child Services / Probation office within identified time frames.
Requirements
- High School diploma or GED required
- Some experience in a Human Services field preferred
- At least 21 years of age
- Valid driver's license and auto insurance with 6 or less points on driving record
Qualifications
- Comfortable working evening shifts, with a minimum commitment of four nights per week
- Willing and able to transport clients of all ages using their personal vehicle
