Family Success Network Family Coach

About the role

Child & Adolescent Behavioral Health is seeking a full-time Family Success Network Family Coach to support children, youth, and families in meeting life's challenges. As part of a collaborative network, this role involves providing direct services such as administering screenings, conducting home visits, facilitating parent education programs, and offering financial literacy training.

Responsibilities

• Provide ongoing supportive consultations and prevention case management services via various communication methods.
• Administer universal screenings for trauma, substance use, and mental health, using tools like ACEs Survey, CTAC, UNCOPE, and a mental health screening tool.
• Offer short-term, intensive in-home services, including a series of monthly home visits and access to the six pillars of service delivery.
• Develop and implement prevention services plans, utilizing evidence-based programming to enhance protective factors and family stability.
• Facilitate parenting programming and Life Skills training, incorporating Motivational Interviewing to support behavioral change.
• Engage families in developing and working on family success plans, providing financial literacy support, and utilizing trainings such as Triple P, Transition to Independence Process Model, Motivational Interviewing, and Strengthening Families Framework.
• Manage an ongoing caseload of up to 30 families, attending meetings with collaborative members, and maintaining relationships with local social service partners.
• Document services and submit regular programmatic and fiscal reports to the Program Manager and other partners.
• Represent the organization at required trainings and meetings, and serve as a point of contact for the county of service.

Requirements

• A minimum high school diploma, with a college degree preferred.
• One year of experience working with families.
• A valid driver's license and reliable transportation.
• Flexibility, knowledge of community resources, and teamwork skills.
• The ability to work with diverse populations.

• Experience with evidence-based programming and family-centered approaches.
• Strong communication and interpersonal skills.
• Ability to manage a caseload and coordinate with multiple stakeholders.

Schedule
