Family Services Case Manager

About the role

JusticeWorks YouthCare is seeking an experienced Family Services Case Manager to support families in Denver County, CO.

Responsibilities

• Support families by providing resources, advice, and assistance.
• Connect families with local services such as housing assistance, healthcare, and other support services.
• Provide guidance and direction to parents and families in need, helping them find the resources they need.
• Assess and engage families in service planning based on demonstrated needs.
• Plan and implement appropriate interventions using evidence-based and best practice approaches.
• Schedule and complete intakes within 24 hours of assignment.
• Provide 24+ hours of face-to-face contact per week to meet family needs.
• Transport clients utilizing personal vehicle, as necessary.

Requirements

• Masters or Bachelors Degree required, preferred areas of study include social work, criminal justice, psychology, or related field.
• A minimum of one (1) year direct care experience or supervised practice in lieu of the experience requirement.
• Valid driver’s license, auto insurance, and access to a vehicle.
• Pass current, applicable clearances.
• Pass pre-employment, post-offer drug screening.
• Ability to work non-traditional hours including evenings and weekends.
