Responsibilities
The Family Resource / Family Support Specialist completes the screening and initial assessment interview and enters initial data into the Program Management System on families referred to the program; enters assessments into the Danville-Pittsylvania Community Services and Healthy Families America/Virginia approved Medical Health Record database; assists with and provides presentations to referring agencies within the community.
Key responsibilities include:
- Maintaining and completing all required documentation as prescribed by Healthy Families America standards, licensure, and Medicaid with regard to clinical services and medical records keeping
- Complying with all regulations applicable to program services
- Providing intensive home-based case management services to children at risk of serious emotional disturbance and their families to promote positive mental health development, family functioning, parent-child interaction, ensure receipt of appropriate medical care, and prevent child abuse
- Developing and implementing individual service plans
Requirements
To be eligible for this position, candidates must have:
- A Bachelor's Degree with at least 15 hours of coursework in human services, with at least two years of experience working with children five and under
- Alternatively, a Bachelor's degree in Psychology, Social Work, Vocational Rehabilitation, Counseling, Sociology, Criminal Justice, or other approved Human Service degree
- Certification as a Psychiatric Rehabilitation Practitioner in Virginia is also acceptable
- A valid driver's license and safe driving record
Additional requirements include:
- Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request
