Family Practice/Primary Care Nurse Practitioner
What You ll Do
- Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities.
- Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership.
- Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
- Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
- Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
- Utilize Bloom's integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.
- Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.

Compensation
$100,000 $125,000 annually, depending on experience, with opportunity for performance-based bonuses and full benefits.

Qualifications
- Nurse Practitioner or Physician Assistant with a master's degree and active Colorado licensure
