Family Physician - Defiance, OH

About the role

We provide high quality, proactive primary care to adults and children in partnership with local employers or unions. We focus on prevention and wellness, acute and chronic disease management, and maximizing provider-patient relationships.

Responsibilities

• Provide patient-centered primary care, to employees at assigned Marathon Health Centers through an Employer-Sponsored Direct Primary Care.
• Enhance relationships with patients by facilitating preventive care and proactively working with patients to prevent diseases and injuries.
• Align incentives with improved health outcomes, driving patient engagement, and creating an atmosphere where patients are highly satisfied with high touch service from their care team.
• Focus on preventive care and utilize electronic medical records and rigorous population management tools to assist in providing state-of-the-art care.
• Collaborate with at least two or more Advanced Practice Providers (APPs) within the organization to ensure interdisciplinary teamwork and improve quality of care.

Qualifications

• Graduation from an accredited medical school and completion of residency in Family Practice or Internal Medicine + Pediatric.
• Board certification or eligibility and intent to certify required; Active state license required; DEA preferred.
• CPR/BLS certification required at time of start date. Per state regulations, may be required to have a dispensing license.

Pay and Benefits

• Pay Range: $195,000 - $238,000/yr for a schedule of 34hrs a week.
• The actual offer may vary dependent upon geographic location and the candidate’s years of experience and/or skill level.
• This position is also eligible for an annual incentive.
