Family Nurse Practitioner: Sign on Bonus of $10,000

Position Summary

Under the administrative and clinical supervision of the Medical Director through a written supervision agreement, performs various patient care, physician support duties and tasks as required.

Essential Functions

• Provides care services including, but not limited to, initial evaluation, physical examination and limited medication management, coordinating care with other providers, ordering labs and EKGs and obtaining information on adverse events
• Provides addiction treatment services and some mental health services to patients at a clinic(s)
• Provides continuity of care by arranging follow-up appointments
• Administers medications
• Obtains prior authorization for services, provides assessments and other testing as needed
• Prepares reports and participates in agency quality assurance activities

Qualification Requirements

• Current State of Connecticut certification and/or license as an Advanced Practice Registered Nurse
• Active DEA controlled substance registration and an active Connecticut controlled substance registration

Compensation & Benefits

• Salary Range: $110,000 - $150,000 with a $3,000 sign on bonus!
• Actual pay will be determined based on several factors. These may include education, work experience and in some instances, certifications.
