Responsibilities
Clinical Care & Patient Services
- Conduct patient assessments, physical exams, and diagnostic evaluations
- Diagnose and manage acute and chronic conditions across the lifespan
- Develop and implement individualized treatment plans
- Prescribe medications and perform procedures within scope of practice
- Provide preventive care, screenings, and health maintenance services
Patient Education & Care Coordination
- Educate patients and families on disease management, prevention, and wellness
- Collaborate with care teams to ensure coordinated, patient-centered care
- Support transitions of care and appropriate referrals within and outside the network
Quality & Population Health
- Participate in quality improvement initiatives and care gap closure efforts
- Support population health strategies and value-based care programs
- Monitor patient outcomes and contribute to continuous improvement efforts
Documentation & Compliance
- Maintain accurate and timely documentation within the Electronic Health Record (EHR)
- Ensure compliance with HIPAA, regulatory, and organizational standards
- Support coding, documentation accuracy, and billing requirements
Requirements
Required
- Master’s or Doctoral degree in Nursing from an accredited program
- Current, unrestricted Indiana Nurse Practitioner license
- Board certification as a Family Nurse Practitioner (FNP)
- Prescriptive authority and DEA licensure (or ability to obtain)
- Strong clinical assessment, diagnostic, and decision-making skills
- Excellent communication and collaboration skills
Preferred
- Experience in primary care, community health, or FQHC settings
- Experience working with diverse or underserved populations
- Familiarity with EHR systems such as Epic
- Experience in team-based or integrated care environments
