Family Nurse Practitioner

Responsibilities

• Provide home-based primary and palliative care to medically complex, home-limited patients.
• Conduct in-home and virtual visits, perform comprehensive assessments.
• Manage chronic and acute conditions, develop patient-centered care plans.
• Collaborate with an interdisciplinary team including physicians, nurses, social workers, and care coordinators.
