Responsibilities
- Provides clinical services within the scope of a Family Practice Nurse Practitioner.
- Diagnoses, treats, and provides high-quality patient-centered care for medical conditions in children and adults following accepted medical standards of practice.
- Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
- Refers patients to specialists and to relevant patient care components as appropriate.
- Evaluates and revises patient care/treatment plan with patients and family as necessary to achieve improved health.
- Provides education for patients and/or families about preventative care, medical issues, and use of prescribed medical treatment.
- Provides accurate documentation of patient care, documents orders, directives, procedures, and patient responses in the medical record, and communicates them to other members of the health care team; maintains accurate, legible, timely, and confidential records.
- Participates actively in the advancement of FCP’s Medical Home model, including innovative ideas, functions as leader of a Care Team, participates in care team clinical benchmarking, and meaningful use of the electronic health record.
- Collaborates with the Medical Director, Provider Team, and Nursing Director to identify areas for system improvement, increased efficiency, communication, and quality improvement activities.
- Develops relationships to provide for a broad spectrum of care with various ancillary specialties (e.g., Rehab, Dental, Optical, Community Health, Behavioral Health).
- Communicates within the department as well as across departments to ensure information is shared for effective operations, and maintains strict confidentiality and adherence to HIPAA policies and procedures.
Requirements
- Master’s Degree or PhD in Nursing with an emphasis on family practice from an accredited college or training program recognized by the State of Wisconsin.
- Possesses and maintains unrestricted licensure provided by the State of WI as an Advanced Practice Nurse, BLS certification, and unrestricted DEA licensure.
- Must possess proof of applicable health department testing and vaccinations or testing/vaccination will be provided by FCP Employee Health.
- At least 3 years of clinical practice in Family Practice, urgent care, or emergency medicine.
- Must successfully pass all applicable background checks and drug screens.
Preferred Qualifications
- Family Practice experience preferred.
- Excellent patient relations and communication skills.
- Ability to participate in clinic committees as requested.
Background Checks and Screening Requirements
- Basic Background and Drug Screen (National Federal Criminal, State Criminal, County Criminal, National Sex Offender, Education Verification, Previous Employment Verification).
- DOJ Caregiver (Health).
