About the role
Community Healthcare Network is a Federally Qualified Health Center (FQHC) serving underserved communities in Brooklyn, Bronx, Manhattan, and Queens. We provide care at our health centers and through various programs, touching the lives of 85,000 people annually. This full-time position is based at our South Bronx Health Center located at 94-98 Manhattan Avenue, Brooklyn, NY 11206. The Family Nurse Practitioner will report to the Medical Director and be an integral part of a collaborative care team, including nursing staff, Health Education, Nutrition, Social Work, and Behavioral Health.
Responsibilities
- Provide evidence-based primary care to patients.
Requirements
- Valid Family Nurse Practitioner N.Y.S. License.
- At least two (2) years of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
