About the role

Community Healthcare Network, a Federally Qualified Health Center (FQHC) serving underserved communities across Brooklyn, Bronx, Manhattan, and Queens, is seeking a Full-Time Family Nurse Practitioner for our South Bronx Health Center located at 81 W. 115th Street New York, NY 10026.

Responsibilities

• Provide evidence-based Primary Care to patients
• Serve as an integral member of the collaborative care team, working alongside nursing staff and other services including Health Education, Nutrition, Social Work, and Behavioral Health

Requirements

• Valid Family Nurse Practitioner N.Y.S. License
• At least two (2) years of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting
