About the role
The Nurse Practitioner/Physician Assistant at Community Health Centers plays a crucial role in delivering comprehensive primary care services in ambulatory settings. They collaborate with the health care team to improve patients' health and enhance their quality of life.
Responsibilities
- Provide outpatient comprehensive primary care service including preventive and health maintenance services, acute and chronic care, clinical evaluation, diagnosis, and treatment.
- Collaborate with the health care team to develop and implement care plans for patients.
- Conduct routine health screenings and assessments.
- Manage patient medical records and ensure accurate documentation.
- Participate in ongoing education and professional development activities.
- Assist in the coordination of care with other healthcare providers.
Requirements
- Education: Master's degree or Post- Master's degree certification (for applicants graduating on or after October 1, 1998).
- Experience: Three to five years of experience preferred.
- Certification/Licensure: Current certificate issued by ANCC and/or AANP or eligible, active Florida license required, DEA license preferred, BLS certification required.
- Skills: High degree of responsibility, excellent interpersonal skills, organizational ability, problem-solving skills, written and oral communication skills, accuracy, attention to detail, manual dexterity, compassion, empathy, teamwork, respect for dignity, patient care needs, computer proficiency, logical approach to work, meticulous record-keeping, bilingual.

- Education: Graduate from a certificate or currently closed program demonstrating compliance with Board guidelines (for applicants graduating prior to October 1, 1998).
- Experience: Three to five years of experience preferred.
