Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner to provide high-quality, comprehensive primary care to patients across the lifespan.
Key Responsibilities
- Provide comprehensive primary care services, including health assessments, physical examinations, diagnosis, and treatment planning.
- Order, interpret, and evaluate diagnostic tests and develop individualized plans of care.
- Prescribe medications and treatments in accordance with scope of practice and regulatory requirements.
- Deliver preventive care, health screenings, counseling, and patient/family education.
- Collaborate with physicians, nurses, and other healthcare professionals; make referrals to specialty and community resources as appropriate.
- Manage and document patient care accurately and in a timely manner.
- Provide age-appropriate care to pediatric, adolescent, adult, and geriatric patient populations.
- Participate in quality improvement initiatives, committees, and special projects as assigned.
- Provide telephone coverage, home or hospital visits, and on-call support as directed.
- Serve as a preceptor, mentor, and role model for students and staff when applicable.
- Maintain compliance with organizational policies, confidentiality standards, and regulatory requirements.
Qualifications
- Education: Bachelor’s and Master’s degree in Nursing. Graduate of an accredited Nurse Practitioner program.
- Bilingual (Spanish preferred).
- Experience & Skills: Strong clinical experience as a Family Nurse Practitioner. Excellent clinical judgment, problem-solving, and leadership skills. Effective organizational, communication, and interpersonal abilities. Basic computer proficiency.
Licensure & Certifications
- Licensure & Certifications: Current New York State Registered Nurse License and Registration. Current Nurse Practitioner Certification and Registration. Active collaborative practice agreement, as required by NYS law. Certification (or eligibility) by a recognized specialty organization. Current BCLS certification; Infection Control and Child Abuse training.
