Family Model Provider

About the role

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are seeking a Family Model Provider to open their home to an individual with developmental and/or physical disabilities.

Responsibilities

• Provide a safe and comfortable home environment
• Ensure the basic physical and emotional needs of the individual are met
• Establish a comfortable, supportive, and genuine relationship with the individual
• Offer a 24/7 family-like setting for the individual

Requirements

• High School Diploma/GED
• Valid Driver's License
• Previous experience working as a Direct Support Professional
• Open to supervisory visits in your home

Qualifications

Must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support.

Skills

Caring, compassionate, and reliable skills are essential for this role.

Benefits

• Contracted and reimbursed at a per diem rate PLUS monthly room and board from the individual they support
• The income is tax-exempt
