About the role
We are hiring for a Family Model Provider Type: Family Model Provider (TN) - Independent Contractor. Independent Contractors are not employees and are not eligible for company benefits.
Responsibilities
- Seeking to understand the individual in the context of their personal history, their social and family networks, and their hopes and dreams for the future.
- Serving the individual in eating nutritious meals and snacks consistent with dietary needs, resting (by providing sleeping accommodations), daily self-care activities, leisure/recreation, education and working.
- Supporting the individual in food preparation, and cleaning of the home.
- Honoring the choices, preferences and culture of the person you support.
- Aiding in the provision of opportunities for social, emotional and spiritual needs to be met.
- Aiding in providing transportation and scheduling of all medical, therapy, vocational and recreational activities.
- Ensuring that the home environment is safe and in good condition.
- Implementing services and supports to address goals identified through the Person Centered Plan.
- This includes medical, psychiatric, recreational, spiritual, and emotional needs.
- Aiding the person supported by attending treatment team meetings and facilitating communication related to individual needs, response to service and supports, and progress toward goals.
- Participating in supervision as agreed upon in the RHA supervision contract.
Requirements
- High School Diploma/GED
- Must be 18 years old or older
- Documentation of home ownership: current mortgage statement or renter’s lease
- Valid Driver’s License; Reliable transportation with valid vehicle insurance
- Ability to pass criminal background and Registry checks as well as a Drug Screen
- TB Screenings for all members of household
- Work as an Independent Contractor
- Pre-employment screening: Complete criminal backgroundName checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
- Drug testing
- Education verification and other credentialing based on position requirements
- Proof of employment history or references (if required)
- Positions that require driving: Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
