Family Mentor- Family First Prevention Services

Essential Functions Provides for the safety, stability and well-being of children and families during the referral period Delivers culturally competent services using a strengths-based approach to children and families consistent with the evidence-based practice models Meets DCF timelines to connect with the family and facilitate the initial family meeting Engages and works in partnership with the case manager and family to complete assessments Responds promptly to family crisis situations Meets with clients at least once a week to offer one-on-one support Develops recovery plans, coping methods and rehabilitation services with the client Facilitates treatment assessments and referrals and provides client support in arranging for medical appointments, counseling, support groups, and other resources as needed Monitors the client's behavior, schedule, medication, and activities and reports any unusual behavior, conflicts, or recovery deviations to the relevant professionals Provides transportation and resources and coordinates community support services as needed Job may require the employee to be able to regularly drive to various DCCCA locations and other business locations throughout the day or night as required Other duties as assigned Education, Experience, And Licensure A high school diploma or equivalent is required Must have lived experience with substance abuse, treatment, and recovery with a minimum of two to three years of sustained recovery; Working with an active recovery program (12-step, church/spiritual, other recovery support group) preferred Requires knowledge of recovery resources and has experience interacting with individuals seeking recovery Personal experience with child welfare preferred Must possess a valid state Driver's License, in force automobile liability insurance, and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by DCCCA policy. Availability of reliable transportation is required for this position. EOE
