Family Medicine Physician

About the Role

We are seeking a Family Medicine Physician to provide relationship-driven, quality-focused outpatient primary care for a senior population in a supportive team environment.

Responsibilities

• Manage a small patient panel (~537 patients).
• See 15–18 patients daily.
• Provide comprehensive outpatient care with a focus on high-quality outcomes.
• Participate in minimal call duties.
• Deliver patient-centered services and contribute to quality improvement initiatives.
• Maintain a manageable patient load to ensure thorough care.

Qualifications & Skills

• Licensed physician with a passion for primary care.
• Strong communication skills and experience in value-based care models.
• Ability to build long-term patient relationships.
• Collaborative team player.
