Family Medicine Nurse Practitioner (NP)

About the role

Outpatient Primary Care clinic just north of Palm Beach is seeking a Family Medicine Nurse Practitioner with prior Value Based Care experience (HEDIS, star ratings, care gap closure). The role focuses on a senior patient panel, emphasizing quality outcomes, with a small panel size allowing 15–18 patients per day and minimal phone call responsibilities.

Responsibilities

• Provide primary care services to a senior-focused patient panel
• Manage and close care gaps to meet HEDIS and star rating metrics
• Deliver value‑based care aligned with quality outcome goals
• Maintain accurate documentation and follow‑up in an outpatient setting

Requirements

• Current Nurse Practitioner license (Family Medicine or Adult‑Gerontology)
• Demonstrated experience in Value Based Care, including HEDIS reporting, star ratings, and care gap closure
• Ability to work Monday–Friday, 8 am–5 pm with no nights or weekends

Pay

Base salary range: $125,000 – $140,000 per year, with potential for up to 20% bonus.

Schedule
