Family Medicine Nurse Practitioner

Responsibilities

• Autonomously lead patient assessments, diagnostic evaluations, and preventative screenings while collaborating with physicians to diagnose and treat complex acute and chronic health conditions.
• Counsel patients and families on preventive wellness and treatment options while managing direct specialty referrals, community resource coordination, and rigorous electronic health record documentation.

Requirements

• Licensure / Certification / Registration: Required Credential(s): Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• One or more of the following: Nurse Practitioner credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date. Registered Nurse credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date.
• Preferred Credential(s): BLS Provider. American Heart Association or American Red Cross accepted. Drug Enforcement Admin credentialed from the Drug Enforcement Administration.
• Education: Master's degree of Nursing required.

Qualifications

• Hands-On Nurse Practitioner work experience is required.
