About the Role
This is an exceptional opportunity for a motivated advanced practice provider to join an employed position within a busy, well-respected practice, supported by a strong referral base and the full resources of a robust health system network. Join an established team of 2 APPs
Call Structure
Call structure: 1 week per month, Monday through Sunday
Practice Setting
Practice setting: Outpatient Primary Care office
Access to Resources
Access to 8 total exam rooms (2 dedicated rooms per provider) utilizing the Athena EMR system
Employment Status
Employed position within a nonprofit health system
Board Certification
Board Certified or Board Eligible in Nurse Practitioner Eligible for state medical licensure and DEA registration
Enhance Patient Access
Drive clinical excellence by performing daily comprehensive chart prep, managing an expected volume of 15 to 20 office patients, and efficiently evaluating lab and imaging results
About the Facility
Focus: Everyday healthcare, preventive screenings, and management of chronic conditions.
Services: Routine physicals (including sports physicals), preventive health exams, vaccinations (such as flu shots), and lab testing.
Serving the local city population of Brazil, Indiana, which is approximately 8,350, as well as the surrounding community of Clay County, which has a total population of approximately 26,400

Minimum Requirements
Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Nurse Practitioner specializing in Prescriptive Authority credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date required.
Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
Education: Master's degree of Nursing required.
