About the role
This is an exceptional opportunity for a Nurse Practitioner or Physician Assistant to join a busy, well-established outpatient rural primary care practice with a strong referral base and the full support of a robust health system network, conveniently located on the campus of Ascension Calumet Hospital.

How you’ll make an impact in this role
Join an established team of 1 physician and 5 APP's
Call structure: 1:7
Practice in a high-volume rural primary care clinic with a supportive, team-oriented environment within a busy and collaborative setting with an average of 12-15 patients per day
Located in the scenic Chilton, WI area. Enjoy small-town charm with easy access to Appleton, Green Bay, and outdoor recreation

Minimum requirements
- Licensure / Certification / Registration: Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
- Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.
- Education: Master's degree of Nursing required.

- Experience in rural healthcare settings
- Experience with electronic health records (EHR)
