Responsibilities

• Autonomously lead patient assessments, diagnostic evaluations, and preventative screenings while collaborating with physicians to diagnose and treat complex acute and chronic health conditions.
• Counsel patients and families on preventive wellness and treatment options while managing direct specialty referrals, community resource coordination, and rigorous electronic health record documentation.

Requirements

• Required credential: Nurse Practitioner credentialed from the Wisconsin Board of Nursing OR Registered Nurse credentialed from the Wisconsin Board of Nursing OR Nurse Practitioner credentialed from the Minnesota Board of Nursing OR Registered Nurse credentialed from the Minnesota Board of Nursing (obtained prior to hire or transfer date).
• Education: Master's degree in Nursing required.
• Experience: Minimum of 2‑3 years of family medicine and/or walk‑in style clinic Nurse Practitioner experience required.

Qualifications

• Preferred: BLS Provider (American Heart Association or American Red Cross accepted).
• Preferred: Drug Enforcement Administration (DEA) credential.
