About the Role
This is a 100% remote opportunity where you'll provide ongoing primary care to established patient panels, maintaining continuity of care while leveraging seamless access to specialists, referrals, and Epic-based medical records. The role emphasizes longitudinal, relationship-based primary care within an integrated health system.

Responsibilities
- Provide ongoing primary care to an established patient panel
- Maintain continuity of care using integrated health system resources
- Utilize AI-powered tools to assist with patient history review, care gap identification, and documentation
Requirements
- Active licensure in one or more of the following states: New Jersey, Massachusetts, North Carolina, South Carolina, Wisconsin, or Minnesota
- Candidates with multiple state licenses or Interstate Medical Licensure Compact (IMLC) eligibility are highly desirable
- MD or DO
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Passion for delivering patient-centered care through innovative technology
Pay
$250,000.00 - $310,000.00
