About the role

Practice innovative, relationship-based medicine while supporting patients within some of the nation's most respected health systems, including Mayo Clinic, Cedars-Sinai, Mass General Brigham, Hackensack Meridian Health, and Hartford HealthCare. This is not traditional telemedicine. Physicians provide longitudinal primary care as part of integrated health system networks, maintaining continuity of care, access to specialists, referrals, and Epic-based medical records.

Responsibilities

• Provide longitudinal primary care to patients.
• Maintain continuity of care and facilitate access to specialists and referrals.
• Utilize Epic-based medical records for documentation.
• Leverage AI-powered tools for chart review, patient history review, care gap identification, clinical insights, and documentation to reduce administrative burden.

Requirements

• Active medical licensure in Minnesota (MN).
• Multi-state licensure and eligibility for the Interstate Medical Licensure Compact (IMLC) are strongly preferred.

Qualifications

• MD or DO degree.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• Passion for patient-centered, technology-enabled care.
