About the role

This locums position is with a PACE (Program of All Inclusive Care for the Elderly) site in Seaside, OR, serving adults over 55. The provider will work in an outpatient clinic and conduct home visits to assisted living facilities, skilled nursing facilities, or personal residences, often alongside a nurse. The role involves collaboration with an 11‑person interdisciplinary team and primary use of the EMR for patient management.

Responsibilities

• Provide outpatient clinical care to geriatric patients (55+)
• Conduct home visits to assisted living, SNF, or personal residences, typically accompanied by a nurse
• Collaborate with an 11‑person interdisciplinary team within the PACE program
• Manage patient care primarily through the EMR, coordinating visits and community care with team members

Requirements

• Geriatric experience (majority of patient population is over 55)
• Active medical license
• Current DEA registration
• Board certified or board eligible (BC/BE) in Internal Medicine or Family Medicine

Skills

• Basic Life Support (BLS) certification

Schedule

• Outpatient clinic only
• Monday‑Friday, 8 am‑5 pm (flexible 3‑4 days per week considered)
• No call required
• Minimum of 2 weeks per month (ideally full‑time)
• Guaranteed 8 hours per day; overtime after 9 hours
• Start date: November
