Family Medicine Advanced Practitioner

Job Summary

You will provide comprehensive healthcare to patients of all ages, including preventative care, diagnosis and treatment of illnesses, managing chronic conditions, and referring patients to specialists when necessary, essentially acting as the primary healthcare provider for individuals and families across the lifespan.

Responsibilities

• Apply broad medical expertise to address a wide range of health concerns, fostering long-term relationships with patients and guiding them through their healthcare journey.
• Meticulously assess, thoughtfully treat, and effectively communicate to ensure holistic and continuous care.
• Deliver full-spectrum primary care, including preventive care, acute illness treatment, and management of chronic diseases (e.g., diabetes, hypertension).
• Support disease prevention through wellness examinations, health screenings, and immunizations.
• Counsel patients on diet, lifestyle choices, and disease management.
• Order, perform, and interpret diagnostic tests such as laboratory work and imaging.
• Offer guidance, counseling, and education on wellness, disease management, and medication adherence to patients and their families.
• Maintain accurate, timely, and detailed medical records, utilizing EPIC appropriately.
• Document direct treatment for patients, including ordering medications, referrals, and following up on specialist consultations.
• Participate in primary care division activities, including peer review programs, quality metric improvement, and policy development.

• Board-Eligible/Board-Certified
• Active or eligible license in the state of Kentucky
