Responsibilities
- Deliver Family Centered Treatment services in home and community settings, meeting families where they are
- Guide children and families through the four phases of the FCT model to support lasting change
- Develop and maintain individualized Service Plans in collaboration with the Treatment Team
- Conduct assessments and provide therapeutic interventions and psychoeducation aligned with treatment goals
- Care for families in setting goals, building skills, and accessing community resources
- Develop discharge and aftercare plans to promote long-term stability and success
- Maintain timely, accurate clinical documentation in the Electronic Health Record and participate in quality assurance activities
Qualifications
- Minimum of a Master’s Degree in Counseling, Social Work, or a related field
- Must currently hold a provisional/associate license (e.g., LCSWA, LCMHCA, LMFTA)
- Must meet qualifications as a Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis
- Current FCT Certification is a plus, though not required — we'll help you get there!
