Family Centered Treatment Specialist

About the role

As a Family Centered Treatment Specialist, you will provide quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment (FCT) model. FCT Certification is required and must be completed within one year of employment to continue providing FCT services and maintain employment with Health Connect America (HCA).

Responsibilities

• Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
• Complete FCT Checkoffs and other internal trainings within designated deadlines via an e-learning system.
• Deliver FCT services to children, adolescents, and their families, primarily in their living environments, with a family focus.
• Provide 24/7 on-call coverage to a caseload of families and children, with services regularly provided in the home and community at the convenience of the child and family, requiring a flexible work schedule.
• Complete all required Clinical Records and FCT documents with the child and family, ensuring timely entry/upload into the Electronic Health Record according to company standards.
• Develop and maintain the Service Plan in coordination with the Treatment Team, following designated authorization processes.
• Work collaboratively with the identified Treatment Team members to ensure effective communication and coordinated service provision.
• Conduct ongoing revisions of the Service Plan and implement it as required.
• Provide psychoeducation as indicated in the Service Plan.
• Conduct interventions aligned with the FCT model, including facilitating movement through the four phases of treatment and completing all fidelity documents.
• Assist families and children in establishing weekly goals and identifying steps to reach them.
• Make referrals to services and resources on behalf of families and children.
• Develop and coordinate a discharge plan based on treatment, indicating all relevant aftercare needs and plans.
• Access community resources beneficial to client aftercare needs.
• Coordinate and oversee initial and ongoing assessment activities.
• Convene the Treatment Team for Service Plan development.
• Consult with medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers, engage community and natural supports, and include their input in the person-centered planning process.
• Ensure linkage to the most clinically appropriate and effective services, including arranging for psychological and psychiatric evaluations.
• Provide and coordinate behavioral health services and other interventions for youth or other family members with other behavioral health professionals and Treatment Team members.
• Monitor and document the status of the consumer’s progress.
• Participate in Quality Assurance/Quality Improvement activities, including audits, corrections, and designated improvement plans and projects.

Requirements

• Minimum of a Bachelor's Degree in human services or related field.
• Two years’ experience working with youth or families.
• Current Family Centered Treatment Certification is preferred.

State-Specific Requirements

• Mississippi: Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS.
• North Carolina: Must meet qualifications as a Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis.
• Ohio: LSW or eligibility for provisional licensure preferred.
• Virginia: Current Qualified Mental Health Professional (QMHP) or ability to register as a QMHP with the Virginia Board of Counseling.
