Family Centered Early Supports and Services Intake Coordinator

Lakes Region Community Services (LRCS) is seeking a Family Centered Early Supports and Services (FCESS) Intake Coordinator to assist individuals and their families through the eligibility process for FCESS.

About the role

The FCESS Intake Coordinator will facilitate the intake process by communicating with the contact provided on the referral in a timely manner, coordinating with families and co-workers to schedule evaluations, and providing follow-up as needed.

Responsibilities

• Prepare intake documentation
• Collaborate with colleagues to ensure documentation is completed appropriately
• Collaborate with community partners to obtain referral information

Requirements

• Associate degree or certification in education or human services related field required. A combination of relevant work experience and credits toward a degree program may be considered at the discretion of the Agency.
• At least 2 years of experience with children and their families.
• Valid NH Driver’s license, reliable vehicle, and agency-determined minimal motor vehicle insurance coverage.
• Satisfactory completion of background and applicable pre-employment physical capacity screening checks.

Pay

$21.00–$22.00 hourly

Schedule

Full time, 37.5 hours per week
