Family Case Manager
Job Purpose
The CCSD Community Market Case Manager is responsible for providing comprehensive case management services to individuals and families in need of food, housing, supportive services, and community resources at CCSD's Family Support Center. This position focuses on supporting clients in achieving stability and self-sufficiency. Key responsibilities include creating individualized service plans, connecting clients to essential resources, and delivering ongoing support to ensure long-term success.

Organizational Requirements
As a mission-driven nonprofit organization, all employees are required to contribute to organization-wide initiatives that support community impact. This includes participation in up to six Pop Up and Give community events annually. These occur outside of standard business hours and are scheduled on the third Saturday of the month.
Essential Duties and Responsibilities
- Conduct intake assessments to identify clients' service needs and barriers hindering sustainable self-reliance.
- Develop comprehensive individualized action plans with clients that promote self-reliance, outlining achievable goals and strategies focusing on housing, employment, health, and other essential needs.
- Provide continuous case management services by monitoring client progress, reassessing needs, and evaluating plan effectiveness.
- Conduct regular monthly assessments with clients to ensure they are receiving the necessary assistance and support.
- Work closely with clients to motivate and reinforce independence, self-sufficiency, and self-esteem throughout their journey.
- Provide immediate support and intervention during client crises; develop safety plans and connect clients to emergency services when necessary.
- MEDIATE issues with clients that threaten their self-sustainability.
- Aid SNAP program participants in completing applications, collecting required documents, and providing necessary data; connect clients with nutrition education support.
- Assist clients in identifying and accessing community resources, including housing programs, mental health services, employment opportunities, and financial assistance.
- Establish and maintain an effective and timely referral network; facilitate referrals and ensure clients access available resources.
- Identify suitable shelter options and assist clients in securing and maintaining safe, stable housing.
- Aid clients with budget plans and provide guidance on financial literacy, budgeting, saving, and debt management.
- Assess client's employment skills, interests, and goals; collaborate with the workforce case manager and connect clients with job trainings and employment readiness support.

- Ability to use budgeting tools to guide clients in financial planning and life skills development.
- Proficiency in client management and tracking systems for documenting case notes, tracking referrals, and maintaining program compliance (HMIS, Salesforce).
- Experience with data entry and tracking metrics.
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
- Proficiency in Google Workspace including Gmail, Drive, Docs, Sheets, Slides, Google Meets and Calendar.
Education and Experience
- Required: Bachelor's degree in Social Work, Human Services, or a related field, or an equivalent combination of education and relevant work experience
- Required: 3+ years of Non-Profit experience
- Required: 3+ years of experience in intensive case management
- Required: Fluent (verbal and written) in both English and Spanish
- Required: Experience working directly with diverse populations, including vulnerable or at-risk individuals
- Preferred: Relevant certifications in case management and conflict resolution
- Preferred: Familiarity with housing services, wrap-around programs, or self-sufficiency initiatives
- Preferred: Proven track record of managing a caseload and delivering measurable outcomes in client stability and self-reliance
- Preferred: Familiarity with SNAP program procedures and requirements
Work Schedule and Classification
This position is scheduled to work approximately 40 hours per week, Monday through Friday, during standard business hours aligned with CCSD school operating hours. Evening, weekend, event-based, or on-call work may be required depending on program and operational needs. This position is classified as Non-Exempt under the Fair Labor Standards Act.
Travel Requirements
Reliable personal transportation is required to fulfill travel-related responsibilities. The employee must maintain a valid driver's license and meet the organization's insurability requirements. Mileage between assigned worksites during the workday is reimbursed per the IRS standard mileage rate in accordance with TJOP's expense reimbursement policy.
Work Environment and Physical Demands
Ability to sit, use hands and fingers, talk or hear continually. Ability to stand, walk, reach, push, pull regularly and for extended periods of time. Ability to climb or balance, stoop, kneel, or crouch regularly. Must be able to lift, push, pull efficiently and on a regular basis. Ability to work in a fast-paced active environment. Ability to lift and carry up to 50 lbs. Frequently exposed to various controlled and uncontrolled weather conditions.
