About the Role

At Bridgeport Hospital, we are committed to providing quality medical care and treatment that is coordinated and centered on the patient's specific needs. Under the supervision of the Program Director, the Community Health Worker provides community service navigation for Family Bridge families, engaging patients and families to help them navigate and access community services and resources. This role involves education, advocacy, and support to assist individuals in medically underserved communities, with a focus on outstanding customer service and adherence to Yale-New Haven Hospital’s core success factors.

The Community Health Worker will provide services in a home setting, requiring comfort with autonomy and home visiting.

Responsibilities

  • Engages individuals during street/community outreach.
  • Provides education on healthy behaviors during street outreach, advocacy, referral, and support.
  • Encourages healthy lifestyles for individuals, families, and communities through health promotions, outreach, and marketing.
  • Informs individuals and families about resources they would benefit from, refers them to eligible resources, and assists with navigation of the process.
  • Assists clients in accessing health-related services, including obtaining a medical home, providing instruction on appropriate use, and overcoming barriers to care and/or social services.
  • Coaches and assists patients with MyChart sign-up.
  • Continuously expands knowledge and understanding of community resources and services. Facilitates client access to community resources, including housing, food, clothing, financial assistance, life skills providers, and mental health services.
  • Assists clients in utilizing community services, including scheduling appointments and completing applications for eligible programs.
  • Travels extensively to outreach destinations, agencies, and other community locations.
  • Provides patient reminder calls and follow-up calls for appointments and/or referrals to community resources.
  • Documents all client interactions in an electronic database with accurate notes, including face-to-face visits, telephone communication, action plans, and mailed letters.
  • Maintains records of coordination of care, outreach, patient support, and/or care management activities for reporting and tracking purposes.
  • Attends and prepares for scheduled supervision, team meetings, staff meetings, or rounds.
  • Seeks additional supervision or consultation as needed and follows through with supervisory directives.
  • Builds and maintains positive working relations with clients, providers, and agency representatives to ensure comprehensive service delivery.
  • Works collaboratively and effectively with the care team, including patient navigators and clinical staff.

Requirements

  • High School diploma/GED required; college degree preferred.
  • Minimum of 1-3 years’ experience, preferably in health care, human services, or customer service.
  • Valid driver’s license and reliable transportation required.
  • Home visiting experience preferred.
  • Experience with Epic preferred.
  • Community Health Worker Certification preferred or ability to complete within one year of hire.
  • Bilingual (Spanish/other) strongly preferred.
  • Excellent organizational skills and attention to detail.
  • Ability to lift 10-15 lbs.

Additional Qualifications

  • At least two years of experience with program management, community outreach, education, and organizing.
  • Experience and knowledge of public health and community health work.
  • Knowledge of the community and organizations serving target populations.
  • Multicultural and/or bilingual capabilities are highly desirable.

This position is coterminous with grant funding.
