Summary of the Position

Hourly, Full Time position serving youth and families throughout Ontario County are available. Applicant must be dependable, committed, and able to serve as a positive role model for youth in the community, school, and home settings.

Primary responsibilities

  • Initiate, organize, plan, develop and implement direct advocacy services to assigned participants and their families.
  • All service plans will be based on a strength-based approach using the wrap around model.

Qualifications/Requirements

  • High School Diploma or GED is required.
  • Experience in community work and knowledge of community resources.
  • Excellent written and verbal communication skills.
  • Proficient in Microsoft Office 365 is a plus.
  • Experience using an Electronic Health Record (EHR) System is a plus.
  • Ability to work flexible schedule, some evenings and weekends may be required.
  • Ability to work non-traditional hours.
