Responsibilities
- Knowledge of medications and their correct administration for neonates based on diagnosis, age, and weight.
- Assess neonates at delivery and perform neonatal resuscitation.
- Perform head-to-toe assessments on neonates and reassessments as per policy.
- Monitor hemodynamic status of neonates and correctly interpret findings.
- Demonstrate knowledge of cardiac monitoring, identify dysrhythmias, and treat appropriately based on the Neonatal Resuscitation Program.
- Perform and maintain IV therapy in neonates requiring medication administration, fluid, and electrolyte management.
- Monitor respiratory status of the neonate and maintain a patent airway.
- Coordinate and supervise neonatal care while promoting neonatal self-regulatory behaviors.
- Assist physicians with procedures in the NICU/Nursery.
- Interact professionally with neonate/family and involve family in the plan of care.
- Maintain a safe environment, including infant security policy, identification bands, and functioning monitor alarms.
- Demonstrate clinical knowledge of normal values for vital signs and lab work for neonates.
- Float within the Maternal Child Division willingly as per competency matrix.
- Promote and participate in hospital performance improvement activities, including data collection and interdisciplinary team participation.
- Competent in insertion, assessment, and discontinuation of catheters, tubes, and/or drains.
- Respond to code blue in the department, initiate CPR, and demonstrate a calm, professional manner during emergencies.
- Demonstrate knowledge of the Neonatal/Infant Pain Scale (NIPS) for Nursery & Neonatal Pain, Agitation, and Sedation (N-PASS) for NICU.
- Provide comfort measures to infants during painful procedures.
Assessment
- Perform initial assessment of neonates upon arrival to the unit.
- Obtain and document a complete nursing admission assessment.
- Develop an interdisciplinary plan of care upon admission.
- Perform and document accurate systems assessment at the beginning of each shift and reassess as per policy.
- Respond to diagnostic data and initiate appropriate action or intervention.
- Assess family education needs on admission and daily.
- Assess psychosocial needs specific to patient's age, growth, and development.
Planning
- Initiate an interdisciplinary plan of care within eight hours of admission.
- Establish priorities for patient care based on the patient's problem list.
Implementation
- Verbalize understanding of patient diagnosis and interdisciplinary plan of care.
- Safely and correctly initiate and document nursing interventions in emergency situations.
- Perform 24-hour chart check at the beginning of each shift.
- Administer, observe, and document intravenous therapy according to hospital policy.
- Administer medications within 30 minutes of scheduled time, observe and document effectiveness, and report adverse reactions immediately.
- Provide individualized instructions regarding the plan of care.
- Demonstrate safe and correct use of unit-specific equipment.
- Adhere to standard precautions, including hand hygiene guidelines at all times.
- Collect and label specimens per policy when ordered by a physician.
- Perform bedside glucose monitoring according to policy.
- Report all abnormal findings immediately to the physician.
- Insert intermittent or indwelling urinary catheter using aseptic technique.
- Administer oxygen therapy according to physician order.
- Transfer patients in/out of the unit efficiently by obtaining a complete and correct set of physician's orders.
- Complete documentation of patient transfer summary and ensure appropriate referral forms accompany intra-facility transfers.
Evaluation
- Document/report patient responses or outcomes resulting from nursing interventions, treatments/procedures, interpersonal interactions, and patient/family education.
- Evaluate and document patient's progress towards established goals/objectives every shift.
- Update/revise patient care problem list and individualized patient care standards based on patient/family responses, progress towards goals, and/or change in medical status at least every 24 hours + PRN.
- Evaluate patient length of stay and related transfer/discharge planning needs based on anticipated problems, internal or community resources, and support systems.
Professional and Staff Development
- Obtain 100% hospital mandatory required unit-specific in-service annually.
- Plan, present, and document unit in-service as required by level.
- Document competency in clinical skills annually.
- Attend and actively participate in daily huddles.
- Use established lines of communication for resolution of problems and interpersonal conflict.
- Be an active member of a professional organization.
- Demonstrate up-to-date knowledge of FPQC's goals and ongoing initiatives on the Maternal Child Unit.
Leadership
- Participate in the PI program by identifying PI indicators of major aspects of care at the unit level.
- Document and charge supplies used daily.
- Engage in learning opportunities and apply knowledge to practice.
- Actively participate in unit-based council.
Requirements
- Current FL Nursing License.
- Current Basic Life Support - CPR Certification.
- Certification Neonatal Resuscitation Program (NRP must be obtained within 6 months of hire or transfer).
- Graduated from an accredited school of nursing.
- Minimum 1 year RN experience in Level II NICU.
