About the role

Collaborates with Cardiac Rehab clinicians to prescribe exercise according to ACE/AACVPR/ACSM guidelines. Performs duties in a manner consistent with the organization’s mission, vision, values, and standards. Re-evaluates, assists, and encourages members to facilitate compliance with prescribed programs. Maintains a safe and organized facility, identifying and correcting potential hazards. Provides expert knowledge for prescribing exercise to health and at-risk adults and for wellness programs. Demonstrates initiative and responsibility for completing tasks.

Responsibilities

  • Review patient health history in preparation for initial evaluation.
  • Interview patient/family to determine educational needs and establish functional treatment goals and plan for care.
  • Collaborate with the patient’s physician, nurse, and interdisciplinary team members to coordinate the plan of care and communicate patient status.
  • Explain treatment procedures to patient/family before administering.
  • Conduct submaximal fitness assessments and prescribe individualized exercise programs.
  • Formulate exercise prescriptions according to AACVPR guidelines.
  • Utilize proper safety and infection control techniques according to departmental and hospital policy.
  • Operate all department equipment efficiently, including exercise equipment, pulse oximeter, blood pressure cuff, and glucometer.
  • Determine expected outcomes, set priorities, establish goals, and select appropriate interventions for clientele.
  • Evaluate the effectiveness of the exercise prescription for clientele.
  • Evaluate patient objective/subjective response to exercise and modify treatment and plan of care as indicated in departmental policy.
  • Maintain patient confidentiality according to HIPAA.
  • Make appropriate referrals to physical therapy as indicated in departmental policy.
  • Develop a Home Exercise Program based on the patient’s therapy session.
  • Assume a leadership role in maintaining the patient-focused care delivery system at AHH Cardiac Rehab.
  • Ensure patient supplies are available as needed to provide patient-focused care.
  • Operate departmental equipment and unit monitors competently.
  • Perform follow-up phone calls per departmental policy/procedures.
  • Consult with each patient regarding Cardiac Rehab per provider orders.
  • Schedule patient’s initial evaluation at the Cardiac Rehab clinic.
  • Document interaction with every patient.
  • Provide tours of the StrongHearts Rehabilitation Clinic to new patients as needed.

Requirements

  • Bachelor’s degree from an accredited college or university with a major/minor in Exercise Science or Exercise Physiology required. Master’s degree in Exercise Science or related field preferred.
  • BLS certification required within 30 days of employment and prior to providing direct patient care.
  • ACSM certification preferred. Certification must be renewed by the recommended renewal date.
  • Professional certification as an ACSM Certified Clinical Exercise Physiologist or ACSM Registered Clinical Exercise Physiologist preferred.
  • Must be knowledgeable with ACSM/ACE/AACVPR guidelines.
