About the role
Provides hospice care services to patients and families in their homes. Works closely with interdisciplinary teams to coordinate care plans and ensure patient comfort and quality of life.
Responsibilities
- Administer medications and treatments as prescribed
- Assess patient needs and document progress
- Provide emotional and spiritual support to patients and families
- Coordinate with other healthcare providers to develop and implement care plans
- Ensure compliance with hospice policies and procedures
Requirements
- Bachelor's degree in a healthcare-related field
- Current CPR certification
- Experience in home health care or hospice preferred
- Excellent communication and interpersonal skills
Qualifications
- Valid driver’s license
- Ability to travel to various locations within the service area
Skills
- Strong organizational skills
- Effective problem-solving abilities
- Proficient in Microsoft Office Suite
