Evolve Team Lead Coordinator

Qualifications

• Master's Degree in Counseling or other related field
• Full licensure (LCSW, LPC, LMFT, ECT) or associate licensure actively pursuing full licensure (LAPC, LAMFT, LMSW, ECT)

Assigned duties

• oversight of caseload of up to 30 youth ages 16-30 who have experienced First Episode Psychosis within 24 months
• collaborates in a team based approach with other designated staff
• leads weekly team meetings
• provides assessments, ongoing therapy, groups, family, ect. in-clinic and community
• required to meet minimal contacts per month with each client set forth by contract
• provide collaborative documentation
• attend collaborative meetings with staff and other youth serving stakeholders for continuity of care
