Evening shift Clinical Pharmacist

About the Role

The Clinical Pharmacist I serves as a pharmacy generalist in one or more patient care areas. This role involves direct patient care activities and participation in departmental or institutional quality, safety, and cost-saving initiatives.

Responsibilities

• Practice as a clinical pharmacist generalist to ensure appropriate therapy and institutional outcomes.
• Provide patient-centered, interdisciplinary pharmaceutical care.
• Proactively review and process orders accurately and efficiently.
• Prevent and resolve drug-related problems by consulting with prescribers and documenting interventions.
• Perform daily in-depth patient profile reviews to assess, monitor, and recommend appropriate drug therapy.
• Evaluate, intervene, and document clinical interventions according to institutional protocols and guidelines.
• Assess the usefulness of medical literature in optimizing evidence-based care.
• Monitor drug therapy regimens and make recommendations or modifications per protocol.
• Utilize system nomograms and other guidelines.
• Document clinical activities and adverse drug events.
• Attend medical codes as applicable.
• Follow, maintain, and reinforce system policies for proper dosing, storage, handling, dispensing, and administration of medications.
• Handle conflict and ethical issues constructively and appropriately.
• Engage with other clinical pharmacists and leadership for complex clinical situations.
• Use effective patient education materials and techniques for drug therapy, adverse effects, compliance, and administration.
• Participate in coordination of transitions of care.
• Maintain clinical competency through licensure and departmental requirements.
• Serve as a preceptor for pharmacy students or provide at least one educational in-service annually.
• Precept pharmacy students and residents as available.
• Assist in training department staff and other disciplines.
• Participate in research and publication opportunities.
• Engage in departmental mentorship and collaboration programs.
• Recognize process failures and actively engage in analysis and improvement efforts.
• Participate in at least one medication use evaluation or quality improvement project annually.
• Oversee the work of pharmacy students, technicians, and pharmacists to ensure compliance with regulations.
• Document adverse drug reactions and medication variances per policy.
• Follow and reinforce medication use policies for financial stewardship and cost-saving initiatives.
• Engage in review of medication insurance authorization status as applicable.
• Maintain understanding of 340b practices as applicable.
• Perform other duties as assigned or requested.

Requirements

• Eligible for Georgia licensure; must be licensed in Georgia prior to the official start date.
• Basic Life Support (BLS) Certification in accordance with the American Heart Association required within 90 days.
• Advanced Cardiac Life Support (ACLS) required within 6 months if directly applicable to practice specialty.

Qualifications

• One year of relevant experience.
• Pharmacy Specialty Board certification or ASHP-accredited Post Graduate Year 1 pharmacy residency preferred.
• One year of experience will be waived for candidates who have been employed as student/interns within the Emory Healthcare system.

Physical Requirements

• Medium physical demand: 20-50 lbs occasionally (0-33% of the workday), 11-25 lbs frequently (34-66% of the workday), 0-10 lbs constantly (67-100% of the workday).
• Lifting up to 50 lbs maximum.
• Carrying objects up to 25 lbs.
• Occasional to frequent standing and walking.
• Occasional sitting.
• Close eye work (computers, typing, reading, writing).
• Physical demands may vary depending on assigned work area and tasks.

Environmental Factors

• Exposure to blood-borne pathogens, bio-hazardous waste, chemicals/gases/fumes/vapors, communicable diseases, electrical shock, floor surfaces, hot/cold temperatures, indoor/outdoor conditions, latex, lighting, patient care/handling injuries, radiation, shift work, and travel may be required.
• Use of personal protective equipment, including respirators, may be necessary.
